Rehabilitation - How It Works

When you arrive at Calvert Reconnections you will be greeted by our friendly rehabilitation team. Your room will be made up and ready for your arrival. If you haven’t already had a tour round the centre in a previous visit, you will be offered a tour, and shown all the facilities we have on offer.

You will be allocated a key worker, who will be your first port of call for any questions you may have.

Your first 4 weeks will be dedicated to us getting to know you, and you getting to know us. You will be offered a range of different activities to take part in, some of which the clinical team will join in on, and you’ll get to meet a variety of different rehab staff that you’ll be working along with.

Our clinical team is made up of occupational therapists, physiotherapists, and a speech and language therapist. We can either work with a current neuropsychologist, or we can arrange psychology input, including neuro psychology, from Mind Right (https://www.mindright.co.uk/) who offer a range of Clinical Psychologist, Psychotherapist, Neuropsychologist and Psychiatrist services.

We will be looking to get to know you, to establish what you want to achieve during you rehabilitation stay and how we can make that happen.

Whilst the majority of the first 4 weeks of your stay will be activity based, there will be some sessions held directly with the clinical team in the centre. During these sessions, the staff will have conversations with you, will start to establish your goals and create a outline programme, based on what you would like to achieve during your stay. The clinical team will establish a rehabilitation plan, which combines clinical strategies with outdoor activity.

Goal setting process:

Goal example:

I would like to be able to catch the bus independently.

The clinicians would break this goal down into skills (cognitive and physical) required to achieve this goal, such as:

  • Mobility: to walk to the bus stop, to balance on the bus whilst it is moving.
  • Time management: to get to the right location at the correct time.
  • Planning and organisation: choosing the correct route, taking the correct items for the journey, getting on the right bus.
  • Money management: bringing the right money, knowing if you have received the correct change from your journey.
  • Attention: Not missing your stop!
  • Social skills: with members of the public on the bus / the bus driver
  • Communication: explaining where you would like a ticket too.
  • Fatigue management: awareness of fatigue levels, knowing what is a suitable journey length.
  • Decision making: Which bus to get on, where to go, what time is best.

Through a combination of clinical strategies, using outdoor activities to implement these strategies, your rehab programme may look something like this:

  • Hill walking: to work on balance, foot placement on uneven terrain, increase distance.
  • Climbing: to work on decision making skills, safety awareness, balance, attention to task.
  • Planning your day: to establish a routine that suits you best, increase responsibility to manage your time more independently, giving you opportunity to incorporate rest periods to manage fatigue if needed.
  • Attending social groups: interacting with members of the public, money management (paying for the service/class).
  • Food shopping: money management / budgeting (paying for shopping), planning and organisation skills (fitting shopping into routine)
